Who Needs Consent?
Parents/Guardians (or the student themselves if legally an adult) must sign a consent form for their student to receive medical, dental, vision, and other health services at a school-based health center. Services will not be provided without this signed consent.
What Consent Covers?
Typical health services included:
- Routine medical care (well-child exams, sick visits)
- Sports/work physicals
- Immunizations and screenings
- Treatment for illness or injury
- Dental services
- Vision services
- Medication administration
- Other basic healthcare services offered by the SBHC
How Long Consent Lasts?
In the Cincinnati Public Schools (CPS) model, consent for School-Based Health Center (SBHC) services is generally valid until it is revoked in writing. Once a parent or guardian signs a consent form, that consent remains active for as long as the student is enrolled in CPS and receiving care from the same SBHC provider operator. Each CPS SBHC is overseen by one designated provider operator. These

- If a student changes schools within CPS and the new school’s SBHC is overseen by the same provider operator, a new consent form is not required.
- However, if a student transfers to a CPS school whose SBHC is overseen by a different provider operator, the parent or guardian must complete a new consent form specific to that provider.
- Parents or guardians may withdraw or revoke consent at any time by submitting a written request to the SBHC or provider operator.
