Following are the full immunization requirements for preschool:
Four (4) doses are required. If the 4th dose was administered prior to the 4th birthday, a 5th dose is required. If the 4th dose is administered at age 4 or older, no further doses are required.
Three (3) doses of all OPV or all IPV are required. If the 3rd dose was administered prior to the 4th birthday, a 4th dose is required. If a combination of IPV/OPV is received, a fourth dose of either is required.
Two (2) doses of Measles vaccine are required. The first must be administered on or after the 1st birthday. The second dose is required prior to entrance to kindergarten.
Two (2) doses of the Mumps vaccine are required. The first must be administered on or after the 1st birthday. The second dose is required prior to entrance to kindergarten.
Two (2) doses of the Rubella vaccine are required. The first must be administered on or after the 1st birthday. The second dose is required prior to entrance to kindergarten.
One dose of the Hib vaccine is required. This must be administered on or after 15 months of age up to the 5th birthday.
A three (3)-dose series is required for all children prior to entering preschool.
